For existing patients, please inform our staff of any changes to your insurance, address, or contact 302-478-7001 when you make your appointment. Please bring your new insurance card(s) with you to your appointment. Failure to provide these changes will misdirect your bills and can cause you to be directly responsible for payment.
Cancelling
We ask that you provide at least 24 hours’ notice to change or cancel an existing appointment. This will allow us to provide that time to see another patient who needs our services. Repeatedly missing appointments may cause us to terminate our relationship with you since it does not allow our practice to provide you with consistent, high-quality care.
Insurance
We accept most major insurance plans as well as most auto and worker’s compensation.
Prescriptions
Occasionally, we will provide medication prescriptions. Please be advised of the following:
- When it is time for your prescription refill, you need to provide our office with 5 days’ notice to prepare the written prescription.
